DOCTOR'S ORDERS/PROVIDER REFERRAL ONLY FOR DIABETIC SUPPLIES & MEDICAL EQUIPMENT. MEDICATION PRESCRIPTION IN HAND OR EXISTING AT PARTNERING PHARMACY. Emergency assistance provided for durable medical equipment, medication prescriptions, enzyme prescriptions and eyeglasses. Insulin, eye exams and emergent dental care also provided. Do not pay Medicaid co-pays but if Medicaid is pending, they may assist.
